The System

Simple mechanics.
Extraordinary precision.

"Fixed appliances β€” conventional metal brackets β€” remain the most widely used orthodontic appliance globally, supported by the broadest and most robust body of clinical evidence. They are capable of treating the full spectrum of malocclusions with a level of force control that no other system consistently matches." β€” PMC Comparative Analysis of Braces and Aligners: Long-Term Orthodontic Outcomes Β· 2024

Traditional metal braces consist of three components: stainless-steel brackets bonded to the front surface of each tooth, a metal archwire that threads through a slot in every bracket and exerts the force that moves teeth, and elastic ligature ties (or metal ligatures) that hold the wire within the bracket slot. The wire's natural tendency to return to its original shape pulls the teeth β€” gradually, precisely, permanently β€” into their planned positions.

What makes metal braces the most powerful orthodontic tool is the direct mechanical control they give the clinician over every axis of tooth movement β€” tipping, torque, rotation, translation, intrusion, and extrusion β€” all independently controllable through wire bends, bracket prescriptions, and auxiliary attachments. No other system offers this degree of three-dimensional control over tooth position.

Metal braces are not the most aesthetic option β€” but they are the most proven, most versatile, and most affordable. For patients prioritising clinical outcome over aesthetics, or for complex cases where aligner or ceramic options would compromise the result, metal braces remain the unambiguous first choice.

πŸ“š
70+ Years of Clinical Evidence

No orthodontic system has more peer-reviewed clinical data behind it. The outcomes, limitations, and optimisation of metal braces are better understood than any alternative.

🎯
Superior Torque and Angulation Control

Metal brackets provide the clinician with maximum control over root position β€” critical for complex cases where precise three-dimensional tooth positioning determines the long-term result.

πŸ”’
Fixed β€” No Compliance Variable

Metal braces work continuously β€” 24 hours a day, 7 days a week. Treatment outcome does not depend on the patient remembering to wear a removable appliance.

πŸ’°
Most Affordable Fixed Orthodontics

At PKR 60,000, metal braces are the lowest-priced fixed orthodontic system at Hassaan Dental β€” delivering the same clinical capability as more expensive bracket types.

πŸ“
Handles Every Malocclusion

From mild crowding to severe skeletal discrepancy β€” metal braces with appropriate auxiliaries (elastics, springs, TADs) can treat the complete range of orthodontic presentations.

πŸ“Š What the Research Shows
Comprehensive malocclusion treatment: Metal braces achieve correct or near-correct occlusion in the substantial majority of cases across 70+ years of peer-reviewed studies
Superior for complex movements: Torque control, rotation correction, and intrusion are more predictable with metal brackets than aligners in independent systematic reviews
No compliance variable: Unlike clear aligners (which require 22 hrs/day wear), metal braces deliver continuous force β€” outcome is not affected by patient wear habits
Cost-effective: Comparable clinical outcomes to Damon and ceramic brackets at a significantly lower starting price
Retainer compliance critical: As with all orthodontic treatment, long-term retainer wear is essential to prevent relapse regardless of bracket type
Clinical Capability

What metal braces can do β€”
and what the evidence proves.

Metal braces are the reference system against which all other orthodontic options are measured. Their capabilities are not in dispute β€” the question is whether a different system better suits your specific needs.

Why Metal Braces Are the Reference Standard

When researchers want to test whether a new orthodontic system is effective, they compare it to metal braces β€” because metal braces are the established benchmark. Clear aligner trials measure against them. Damon bracket studies compare against them. Ceramic bracket research is judged relative to them. This is not because metal braces are old-fashioned β€” it is because their outcomes are the most thoroughly documented and clinically validated in orthodontics. A patient choosing metal braces is choosing the system with the deepest scientific foundation, not an inferior alternative.

😬
Crowding

Mild to severe β€” including cases requiring extractions. Full arch decompression achievable.

↔️
Spacing

Single or multiple diastemas closed with precise space management and root paralleling.

πŸ”„
Rotations

Superior rotation correction versus aligners β€” particularly for premolars and round-shaped teeth.

πŸ”
Crossbite

Anterior and posterior crossbite correction β€” with or without expansion auxiliaries.

⬇️
Deep Bite

Intrusion of over-erupted teeth β€” precise control via archwire mechanics and bite turbo ramps.

↕️
Open Bite

Vertical bite closure with intrusion/extrusion mechanics β€” combined with habit correction where indicated.

πŸ“
Torque & Root Position

Superior root torque control versus all other fixed and removable systems β€” critical for long-term stability.

πŸ—οΈ
Pre-Surgical Orthodontics

Standard appliance for decompensation before orthognathic surgery β€” precise positioning required.

πŸ”©
Pre-Implant / Pre-FMR

Alignment before crown, bridge, or implant placement β€” opens or closes spaces with controlled root positions.

Candidacy

Who is the ideal candidate
for traditional metal braces?

Metal braces suit a wide range of patients β€” particularly those prioritising clinical outcome, affordability, or case complexity over aesthetics.

βœ… Well suited to metal braces
Children and teenagers β€” the most natural age for orthodontic treatment; aesthetics less critical; faster bone response
Budget-conscious adults β€” metal braces deliver the same clinical result as more expensive options at PKR 60,000
Complex malocclusions β€” severe crowding, multiple rotations, deep bite, open bite requiring maximum force control
Pre-surgical orthodontics β€” decompensation before jaw surgery requires fixed appliances with precise bracket control
Low compliance patients β€” fixed appliance removes the need for patient cooperation in wearing removable devices
Pre-FMR alignment β€” moving teeth into correct positions before crowns, implants, or full mouth rehabilitation
Anyone where aesthetic braces are not a priority and who wants the most evidence-backed orthodontic system
πŸ’‘ Consider an alternative
Strong aesthetic preference β€” ceramic braces or clear aligners offer much greater discretion
Client-facing professionals who cannot tolerate visible appliances β€” aligners or lingual braces may be more appropriate
Mild to moderate crowding with aesthetics priority β€” clear aligners may achieve comparable results less visibly
Sensitivity to the appearance of a "metal mouth" during social or professional events
Active periodontal disease β€” must be treated and stable before any fixed orthodontic treatment begins
Who gets metal braces β€” by age group
πŸ‘§
Children (7–11)

Interceptive phase 1 orthodontics β€” expanders, space maintainers, habit breakers. Metal brackets introduced once permanent teeth erupt.

Phase 1 / Interceptive
πŸ§’
Teens (12–17)

The classic time for metal braces β€” all permanent teeth present, rapid bone response, fastest treatment. Coloured elastics available.

Ideal Age Window
πŸ§‘
Young Adults (18–30)

Metal braces fully effective β€” slightly longer treatment vs teenagers due to mature bone. Most common age group seeking braces.

Highly Effective
πŸ§”
Adults (30+)

No upper age limit β€” adult orthodontics equally effective, may take slightly longer. Often combined with restorative or FMR planning.

No Age Limit
Treatment Journey

Consultation to retention β€”
every step explained.

Metal braces treatment at Hassaan Dental Clinic begins with a complete set of clinical records β€” so the treatment plan is built on accurate data, not assumptions.

1
Orthodontic Consultation & Clinical Records
PKR 1,000 Β· 60 Minutes

Dr. Haris performs a full orthodontic assessment β€” clinical examination, OPG panoramic X-ray, lateral cephalometric X-ray (where required), intraoral photographs, and digital study models or impressions. Cephalometric analysis determines the skeletal pattern and jaw relationship. The complete treatment plan, arch-by-arch tooth movement sequence, expected timeline, and total cost breakdown are presented at this appointment. No brackets are placed until you have approved the plan.

2
Pre-Treatment: Extractions & Disease Control
Where Clinically Indicated Β· Before Bonding

Any teeth identified for extraction to create arch space are removed first, with a healing period before brackets are placed. Any existing periodontal disease is treated β€” gum health must be stable before orthodontic forces are applied. Scaling, root planing, and gum treatment are performed as needed. Any urgent restorative work is completed. Once the oral environment is stable and healthy, bonding is scheduled.

3
Bracket Bonding
60–90 Minutes Β· Painless

Teeth are polished, etched with bonding agent, and each stainless-steel bracket is individually bonded in a precise position β€” determined by the bracket prescription and Dr. Haris's treatment plan. Molar bands may be placed on the back teeth for attachment of auxiliaries. The initial archwire β€” typically a very flexible, heat-activated nickel-titanium wire β€” is threaded through each bracket and secured with elastic ligature ties. The procedure is painless; mild sensitivity and pressure develop over the following 24–72 hours as the teeth begin to respond to the wire.

4
Active Treatment β€” Adjustments Every 4–6 Weeks
PKR 8,000 Per Visit Β· 4–6 Week Intervals

At each adjustment, Dr. Haris removes the elastic ties, takes out the current wire, assesses progress in all three planes, and places the next wire in the treatment sequence. Wires progress from lighter, more flexible initial alignment wires to stiffer, more precise finishing wires. Auxiliaries β€” elastic bands, coil springs, or temporary anchorage devices (TADs) β€” may be added at appropriate stages to correct bite relationships or manage complex tooth movements. Total active treatment: 12–24 months depending on complexity.

5
Finishing & Debonding
Final Active Appointment Β· 45–60 Minutes

When tooth positions, root angulations, bite relationships, and arch coordination meet the treatment objectives, Dr. Haris schedules debonding. Brackets are removed with a debonding instrument β€” each bracket is gently popped off and the bonding resin polished from the tooth surface. Final clinical photographs and digital scan are taken to document the completed result.

6
Retainers β€” Non-Negotiable
Same Day as Debonding Β· PKR 10,000 / Arch Β· Lifelong Night Wear

Retainers are provided on the same appointment as bracket removal β€” the biological window for relapse begins immediately. Options include a fixed bonded retainer (a thin wire glued to the back of the front teeth β€” permanent, maintenance-free, patient does not need to remember anything) or a removable clear retainer worn nightly. Many patients have both. Relapse is biological β€” it will happen without retainers, regardless of treatment quality or bracket system. Retainer wear is lifelong for night-time use.

Important: Unit prices remain the same; final treatment cost may vary after clinical examination. Total monthly adjustment fees depend on treatment duration β€” estimated at your consultation. Any additional procedures required before or during treatment (extractions, periodontal care) are priced separately and disclosed before commencement.

At Hassaan Dental Clinic, Bahria Enclave, Islamabad, traditional metal braces start from PKR 60,000 for the full treatment on both arches. Monthly adjustment visits cost PKR 8,000 per visit. Retainers after treatment are PKR 10,000 per arch. Initial orthodontic consultation including OPG X-ray costs PKR 1,000. Unit prices remain the same; final treatment cost may vary after clinical examination. The total cost depends on treatment duration β€” estimated at your consultation based on case complexity.
Treatment duration at Hassaan Dental Clinic is typically 12–24 months depending on case complexity. Mild cases with minor crowding or spacing: 12–15 months. Moderate crowding with bite correction: 16–20 months. Complex malocclusions β€” severe crowding, open bite, deep bite, or significant skeletal component: 20–28 months. An accurate, case-specific timeline estimate is provided at your consultation after reviewing your clinical records. Biological response also varies between individuals and affects timing.
It depends on the case. Metal braces are superior for: complex tooth movements (rotations, torque, deep bite correction), cases requiring extraction and space closure, pre-surgical orthodontics, and patients who cannot comply with 22-hour aligner wear. Clear aligners are superior for: aesthetics during treatment, mild to moderate crowding in motivated adults, and patients who need to remove the appliance for professional or social reasons. Metal braces have a broader and longer evidence base. Dr. Haris will recommend the most appropriate system for your case β€” not the most expensive option.
The bonding appointment itself is completely painless β€” no injections, no drilling. In the 24–72 hours after bonding (and after each subsequent wire change), most patients experience a dull ache or pressure as the teeth respond to the wire. This is normal and expected β€” it indicates the braces are working. Over-the-counter pain relief (paracetamol or ibuprofen) is effective. Soreness diminishes significantly after the first 2–3 adjustment appointments as the teeth approach their planned positions and the forces required to move them decrease. Ulceration from brackets rubbing on the inner cheek is common in the first 1–2 weeks β€” orthodontic wax is provided to protect the soft tissue.
With metal braces, avoid: hard foods (raw carrots, apples, hard candies, nuts, ice) that can dislodge or break brackets; sticky foods (toffee, chewing gum, sticky sweets) that wrap around brackets and pull them off; and chewy breads or meats that place heavy stress on the wire. Cut hard foods into small pieces and chew with the back teeth. Sugary and acidic foods should be limited generally β€” brackets create more areas for plaque to accumulate, increasing decay risk if oral hygiene is inadequate. Dr. Haris provides a full dietary guidance sheet at the bonding appointment.
Yes β€” always. Retainers are not optional. Teeth are held in their new positions by bone that is still remodelling after treatment. Without retainers, relapse begins immediately after bracket removal β€” particularly in the front teeth. At Hassaan Dental Clinic, retainers are provided at the same appointment as bracket removal. Options include a fixed bonded retainer (a wire glued to the back of the front teeth β€” permanent and maintenance-free) or a removable clear retainer worn nightly. Nighttime retainer wear is recommended lifelong regardless of how long braces were worn. Retainers cost PKR 10,000 per arch.
The classic and most efficient age for metal braces is 12–15 years β€” when all permanent teeth have erupted, the bone is still actively growing and more responsive to orthodontic forces, and treatment tends to be faster. However, there is no upper age limit for metal braces. Adult orthodontics at 30, 40, or 50 is entirely possible and effective β€” it may take slightly longer than in teenagers, and any pre-existing bone loss from periodontal disease must be managed first. Dr. Haris treats patients of all ages and will assess the appropriate starting time at your consultation.
